Also, this is the recommended card (by Oculus and others) that supposedly works the best. I believe the key is you want the "Fresco" chipset, as other are known to be issues, specifically the following:

- ASMedia USB 3.0 eXtensible Host Controller

I had this issue too and resolved it by buying a PCI-usb card.
I’ve read that motherboard USB 3.0 just doesn’t have enough juice to support what the Rift S needs.
A PCI-usb card gets a direct line to the power supply without having to split it with other resources like the usb slot on the motherboard.
Here is the one I bought, may be worth a try for $20. It totally fixed my VR issues.

Last things I can think of are these:
Update the BIOS on your motherboard. This often has fixes for the USB controllers on the board.

When I installed the hardware and booted up the first time, I didn't see any new drivers for it. So I installed the provided drivers from the website. I got the exact card Oculus recommends. Tomahawk is a great mobo, although $142 is too much. It normally sells at $115 range.
I've seen reports of this motherboard USB controllers having issues with Rift S, so you might wanna pick up this USB expansion card if you decide to pick up Rift S.

Motherboard USB ports are often not to spec - yes - even the high end motherboards we all have as enthusiast gamers. The biggest issue as it relates to the Rift S is power - The Rift S needs every bit of specced power from a port to run five cameras, the display, audio and assorted sensors. Often times the boards simply don't provide it.
Since you're having an atypical experience it's probably caused by something on your specific machine with the USB ports often being the culprit.
Personally, I wouldn't get a powered USB hub - there are too many and they often suck too - I'd go with a USB controller that a bunch of us use:

It could simply be the mobo USB ports. Thus issue has been the bane of Oculus headsets since the CV1. This is the most compatible card, recommended by Oculus themselves...

Will also add once you get the USB ports sorted don't use them for rift again as Mobo doesn't have adequate power delivery. Buy one of these and use this for rift instead:

When I first got my Rift S, I had a myriad of problems that were all solved with that little guy. External USB still uses your inboard usb controller and unfortunately that seems to be the issue for Rift S, also power but that one I linked gets its power from a data power connector so it's got enough juice.

So since you tried all of that just get this USB 3.0 card here. USB 3.0 card I used that one for my oculus VR headset and it works great. Just don't use the software CD or it will make it stop working right. That PCI USB 3.0 card does require a SATA power connection from your power supply cables as you can see. For me it has been working for almost a year now with my VR and it's been very stable. When you install that PCI card into your computer. Just start up your computer then windows will automatically install the driver for you. Generally the Software CDs have outdated software that breaks most of the time. Just forget about the CD that comes with it because windows will install the driver for you. Anyways i hoped this information helped you.

This is the card I have. Btw, I also have a dedicated vr USB port on my motherboard but this or any port connected caused the screen to jump and sometimes disconnect. It may work on some motherboards and not have any problems but IMO, it's nice to have a dedicated power source and connectivity for a vr headset for that cheap. It ran the rift with 3 sensors and never a single disconnect.

If you have the Inateck card, trying installing it by the Instructions below, and see if that helps.
Just install the card, plug in the Sata power cable to the card, and let windows install the card for you, don't use the drivers for the card. After the card is installed, you want to install the generic USB drivers, go into the Device Manager, Double click on Universal Serial Bus controllers and right click on Fresco Logic USB 3.0 eXtensible Host Controller, hit update drivers, then Browse my computer for driver software, then Let me pick from a list of available drivers on my computer, then pick USB xHCI Compliant Host Controller. After you've done all that, it should ask you to reboot. Then you should be good to go.
I have the Inateck 7 port version, and it works fine after a reboot, but it's hit and miss after you shut down and restart Oculus Service.
The Inateck 4 port card is the one recommended the most by people. I didn't know this, when I ordered my 7 port card. The 7 port card has 2 different chipsets on it. I'm hoping that the second chipset is in some way, making the card unstable, and this 4 port card, which only has 1 chipset, will work properly.

I didn’t have the same issue, but was having some other issues that started when I got my rift. I ended up buying a usb card using the oculus rift suggested chipset and my problems have been solved. The card I purchased in 2018 is on amazon
May not fix your issues, but it may be worth $20 to try it. Not sure if the rift S uses same chipset - probably best to check oculus website for recommendation.

Same boat here. All my specs are above minimum, and I have plenty of USB 3.0 ports. The VIVE Link Box worked for me, and it even includes Bluetooth.
The Inateck PCIE USB 3.0 Hub also worked, but not as well as the Link Box, which has its own power source. Inateck is recommended by both HTC and Oculus, but works just as well for WMR.

I actually don't think its power anymore I think its data bandwidth, nothing disconnects it just reports usb error(sensors) or the picture freezes(capture card) but the device led says its recording as normal. Make sure you have an open pci slot for it. It will fit in any of them. You dont happen to know which motherboard model you have do you?
If this sounds like too much you really should get a quest. This is what PCVR is right now, and due to the nature of pc’s and pc gaming, its not going to always be plug and play.

It has been vetted and works great with VR headsets like the Rift. I have one, and its the only way I can use VR with my PC (even after a motherboard upgrade). NOTE: DO NOT get the 5 or 7 port models, as they have different controllers and do not work as well. This ought to work, Inateck PCI-e to USB 3.0 (4 Ports) but it seems you are having usb driver issues. Try using windows drivers rather than the product specific drivers. Oculus has problems with non-generic drivers sometimes

Ideally keep the 2 front sensors on USB3, and put the rear sensor on USB2. Its unlikely you'll notice a difference if you do it that way. What I would try to confirm is if the MB has more than 1 USB controller. I know you can add an additional backplate with more ports that plugs into an expansion port on the board... but if they all run to the same controller, they are all competing for the same limited bandwidth. Worst case, get this dedicated card that is known to be the best option specifically for the Rift.

Either way, bit too late for troubleshooting that now hah. But yeah, Oculus have spent 4 years exposing lackluster USB quality in motherboards. It seems that's one corner most of them are happy to cut as bandwidth & power requirements have never really been pushed to the limits of the USB version specs.

I currently use the $15-$20 https://www.amazon.com/Inateck-Superspeed-Ports-PCI-Expansion/dp/B00B6ZCNGM here for about 5 years in my server, USB 3.0 has a bunch of protocols to prevent data corruption, as long as you are using a good cable there should be no issues.
Its also got some good reviews using VR, just do not use the driver CD, let windows install the best driver, and I would not use the power cable since it uses the CLAM SHELL style power adapter that are somewhat prone to catching on fire.

The wiki for this reddit lists the Inateck KTU3FR-4P as "the one" PCIe card to get. I don't have any experience with it, and the one I had was an ooooold card I had laying around that sorta mostly works on that old PC so I can't recommend the one I have even if it was still on the market.

I have the same motherboard, the USB chipsets are incompatible with the rift s and will cause you to loose tracking all the time. The motherboard is great, the rift S is just super picky about this stuff. You’ll want one of these expansion cards:

This is what most people use, there are other but you want one like this one as it has the Fresco L1100 chipset which works best. Don’t install the drivers let windows install it’s own for it and plug the sata power in and you should be problem free.

Hopefully, you have a 3.0 port, but if you don't here is the recommended USB 3.0 adapter. My motherboard has 2 different USB 3, chipset makers. One for the USB 3.0 and another for the 3.1 ports.

I have the same problem. I sent my log files and oculus support said that my pc usb controller is not compatible even though oculus app shows green on usb 3.0.
